Descriptions
The Patients of Valley-Wide Health Systems, Inc. have the
RESPONSIBILITY to:

Participate in care decisions,
follow the health-care provider's instructions and care management. If you
have any questions, just ask.

Give full, honest information on all
forms and in conversations. Bring a list of medications and any information
about a chronic condition you may have.

Report any changes to your general
condition, symptoms, allergies, adverse drug reactions, etc.

Inspect all medications received
from the clinic before leaving the building.

Call the emergency phone numbers
provided to you to notify your provider of emergencies.

Keep appointments and be on time. If
you must miss your appointment, call at least 24 hours in advance to cancel
or reschedule.

Treat the staff and other patients
with respect.

Bring insurance information. If your
coverage or company changes, bring the updated information to your
appointment and notify the front desk staff.

Show your insurance card (including
Medicaid ID card, and Community/ Migrant Health Center Card) when you get
treatment.

Report any changes to your address
or phone number.

Pay your bills, copayments and
deductible of insurance promptly. Contact your patient representative by
calling 589-5161 if you have any questions or financial problems.

Follow all rules -- no smoking in
VWHS buildings or on the grounds.

Report pain based on your perception—location,
duration, and frequency.
