Paying for services is often easier than you think. It is our goal to make this as easy as possible for you. Each patient has a Patient Representative to help you decide on the best way for you to pay for your services. This can be done before your first visit to the clinic. Insurance - Contracted and Private         Top

VWHS will bill your insurance company if you give us the following information: complete name and address of insurance company, subscriber number, group number, name of policy holder, and policy holder's signature to assign payment to VWHS and release information to the insurance company. You are expected to pay any deductible, copayment or coinsurance at the time of receiving services. VWHS will bill your insurance company for the balance within the next 30 days. You are responsible for any part of your bill not covered by your insurance.

However, you may choose to pay in full at the time of services, then bill your insurance company for payment. When you pay for services you will receive a yellow copy of the encounter form. Payment will be mailed directly to you by submitting the yellow copy along with a claim form to your insurance company.

Pharmacy Services          Top

Valley-Wide provides a pharmacy for patients who are on the sliding fee scale program. Select drugs are covered by the sliding scale program. You must pay your copayment at the time you pick up your medications. If you need a drug that is not covered by the sliding scale program, you must pay the full cost to the pharmacy.

Refills: Please call the pharmacy 5 days in advance for all refills. This gives the pharmacy staff enough time to process your request.

Valley-Wide encourages patients with private insurance, Medicare, or Medicaid to use the retail pharmacy of their choice.

Sliding Scale Fee Program            Top

Community/Migrant Health Center (C/MHC) and Colorado Indigent Care Program (CICP) are programs with specific income guidelines. Eligibility and assigned co-payments are based on household size and income. An Eligibility Technician will rate specific information and place you in the program(s) should your household qualify. Information needed to apply includes:

(1) One month of recent income verification. This would include copies of paycheck stubs or income verification from your employer. If you are unemployed, an expense sheet will be given to you to fill out.

(2) Social Security cards for each family member (if applicable).

At the time of enrollment, patients who meet the guidelines will receive information on the specific services they are eligible for. Sliding Fee Program cards are issued at the time you are determined eligible. This card must be presented to the cashier/receptionist at the time of each visit. Sliding fee programs are not insurance. Co-payment is expected each time you have an appointment at the clinic or for each prescription drug prescribed by your health care provider.

Medicaid (including Colorado Access and RMHMO)          Top

You must present your Medicaid card at each visit. The effective date needs to be verified each time by the cashier. Medicaid requires that you, or any of your family members who are enrolled on Medicaid, need to formally make a choice about your Primary Care Provider that will be printed on your Medicaid card. If you would like to continue services in our health care system, we encourage you to select “Valley-Wide Health Systems.” If you do not select a Physician, the State of Colorado will select one for you. Valley-Wide offers application assistance for the various Medicaid programs.

Valley-Wide Health Systems and Colorado Access have partnered to offer Medicaid and CHP+ eligible persons a commitment for continued high quality care. Colorado Access is a private, not-for-profit HMO that ranks as Colorado’s largest Medicaid managed care plan. The plan offers enhanced health care access for Colorado’s medically underserved population. You will be seen by the same Valley-Wide Health Systems’ Primary Care Provider whom you know and trust and have access to many additional benefits. Simply ask at any clinic site to learn more about Colorado Access and what they offer you as a Valley-Wide Medicaid eligible patient.

Child Health Plan +            Top

Valley-Wide Health Systems is a provider of this full health care program for Colorado children and youth. Our staff can help you to determine if your child qualifies for the plan and assist you with the application. CHP+ is a health insurance program for children and youth age 18 and under. The plan covers out-patient clinic visits, hospital service, glasses, prescriptions, mental health care and other needed services. Medicare           Top

If you are on Medicare, VWHS will do all the billing for you. It is very important that you give us your Medicare number with the alpha letter. If you have supplemental insurance, we need the subscriber number, group number and name of policyholder.

Self Pay            Top

Patients who do not have insurance or other medical benefits should pay for their services as they receive them. If a patient has trouble paying in full at the time of visit, he or she can meet with the Patient Representative for help in setting up payments.
